Looks like part of it is a blotch over top maybe the photographer's attempt to make it look like smoke. I don't think smoke came out of the barrel of a cap gun, but I can't recall for sure. I had a Lone Ranger "ivory"-handled 6-shooter cap gun when I was a kid, and, from what I can remember, a puff of smoke came out at the hammer from the cap's detonation. I still remember the smell of the explosive powder when it went off!
